HR: Renewal of medical care plan at LUX MED from July 1, 2025

We are pleased to announce that we renewed the medical care with LUX MED as of July 1, 2025. The financing rules for the program remain unchanged.

It has been added to all packages:

  • a so-called liquid cytology test (with referral).

What you need to do, to continue the program?

In order to maintain continuity of medical care for yourself and your registered loved ones, you must complete, sign and submit the original consent available for download at the link

Due to the increase in the amount of the additional payment financed by the employer, providing consent is mandatory for all employees wishing to use services at LUX MED regardless of the type of package they have.

The original of the completed consent should be submitted to:

Documents can be sent by traditional mail or by internal mail through reception desk or the chancellery.

Documents will only be considered in their original form: printed and signed by the employee. Providing a scan/photo or image will not be accepted.

Failure to provide the original by the deadline of June 20, 2025 will result in the closing of the package you have (regardless of its type) from July 1, 2025. It will be possible to reactivate it after 6 months.

What should you do to change your current package?

Due to the change in rates, you can also change your current package. To do this, you must complete, sign and provide the original of the Declaration of change of package available for download

From June 2, 2025, to July 1, 2025, access to the LUX MED patient portal for making independent changes to packages will also be frozen.

 

Guarantee of avaibility

Please note that if the maximum waiting time for a scheduled appointment as specified in the Accessibility Standards is exceeded, you are entitled to a 100% cost refund incurred for health services provided to an eligible person included in the packages. To benefit from the reimbursement, you must first contact LUX MED Call Centre to attempt to book an appointment. Subsequently, if neither the LUX MED facility nor the cooperating facility has an appointment available, LUX MED will issue a special referral. Based on it, you can make an appointment with any doctor and 100% of the costs will be reimbursed by LUX MED. For more information click  PLAPG / PLPGS

Treatment freedom (refund of treatment costs at LUX MED)

We would like to remind you about the possibility of treatment cost reimbursement in LUX MED. In the case of services provided in Medical Centers other than those indicated by the LUX MED Group, the Contractor will reimburse the costs of services up to 650 PLN/quarter/person. The reimbursement will be from 70% to 100% of the value of the invoice presented, depending on the package, but not more than 650 PLN/quarter/person.

Refunds will be made based on the Application Form submitted together with a set of documents. Detailed conditions of the Reimbursement, requirements concerning documentation of costs incurred and the scope and principles of reimbursement are set out in the document  PLAPG / PLPGS

Cancel, don’t block

In case you cannot make your visit, remember to cancel it in advance so that somebody else can use it. The number of missed appointments is one of the indicators that drives up the price of the packages on occasion of renewals.
